Forms & Family Updates

Forms families may need throughout the year

Annual agreement outlining Pluto Day Care's policies and expectations. This form must be reviewed and signed each September.

Review and update your child's emergency contact and medical information annually or whenever changes occur.

Complete this form if you choose to decline the application of sunscreen provided by Pluto Day Care.

Authorize Pluto Day Care to administer prescribed medication according to your healthcare provider's instructions.

Please Note: Pluto Day Care does not routinely administer non-prescription (over-the-counter) medications. Exceptions are made only when the medication forms part of an approved Individualized Medical Management Plan that has been authorized by the Centre Supervisor and is supported by written recommendations from a Canadian physician or nurse practitioner. Examples include fever-reducing medication for children at risk of febrile seizures or other medically necessary treatments. All other medications administered by the Centre must be prescribed by a licensed healthcare provider and accompanied by this completed Medication Authorization Form.
