For families who choose not to provide or authorize sunscreen for their child. This form acknowledges Pluto Day Care's recommendation regarding sun protection and confirms the parent's decision to decline sunscreen application.
Allows parents/guardians to authorize a responsible individual under the age of 18 to pick up their child from Pluto Day Care and acknowledges the transfer of responsibility upon release.

Complete this form when your child requires a prescription medication to be administered during daycare hours, such as antibiotics. Please note that Pluto Day Care does not administer medications intended to treat symptoms of illness, including fever-reducing medications (e.g., Tylenol or Advil), cough and cold medications, or medications for upset stomachs and gastrointestinal issues, unless specifically authorized as part of an approved medical plan.
This form is required for children who have a prescribed EpiPen for the treatment of anaphylaxis. It provides our staff with important information about your child's allergies, symptoms, emergency procedures, and medication requirements.
Complete this form if your child requires an inhaler or puffer while attending daycare. This form outlines your child's asthma or respiratory condition, medication instructions, symptoms to watch for, and emergency response procedures to help keep your child safe and supported while in our care.
